Patient Health Questionnaire-9 (PHQ-9)
The PHQ-9 is a short questionnaire designed to help identify and measure symptoms of depression.
It asks about how you’ve been feeling over the past two weeks, and includes questions about:
Mood and interest in activities
Energy and sleep
Appetite and concentration
Feelings about yourself
Thoughts of self-harm or hopelessness
